I have heard the surgery is about 100% effective. The doctor told me it's a myth that it's only effective for some people. Those people for whom it doesn't work, he says, usually have some other problem on top of it, like arthritis or tendonitis, which is still causing pain.
In any case, don't fear the local. My mom has her hands worked on all the time with locals. The only part that hurts is the needle, and even that doesn't hurt, as they put that ice stuff on you first to numb your wrist. It's a very quick surgery (I think 20 minutes).
Get it done already! You shouldn't have to live with that pain. (Yours is obviously much worse than mine, which only just started back in November.)
