Jyl,
Ok, thiw piece of advise is all from me: tell your pdoc everything whether you are feeling better or not. If I have learned anything about seeing a shrink, it is that when he digs deeper, the more issues come up.
The psychologist from San Fransisco also said that we (patients) are not to try to control a session. It only creates anxiety by worrying about what to say and what to leave out. He said it is the doctor's job to LISTEN to what is said and how it is said. So even though we may think something is important, it may only be a diversion to the real issue.
I know trust is a huge issue for most of us. That is why I have decided I will let my shrink know as much as I can. Since I can't just "blurt it all out" to him, I write or leave messages, etc.
So, yes, try writing again. As soon as you are done, don't hang on to it. Mail it or fax it. It could actually lead to a really important discussion. If not, it could help to lessen the fear of sharing with him/her.
