As long as the water pressure is sufficient, that's not difficult at all. You're right, they just put a pipe up there. The wall may have to be torn up a little, so you might have to either re-tile the front, or re-plaster the back, depending on how they go in. I had to have my pipe replaced, and they went in through the exterior wall (stucco) since I didn't want the tile damaged. I just ended up with a hole to patch.
There are also those jobbies where you just attach a shower head right on the tub faucet, on a long flexible tube. Then you don't need the pipe put in.
You also need a rack put up for a shower curtain, if it doesn't have any enclosure. Then you're set.
