If he's been going to that many specialists, I'm sure they've tried triptans. Imitrex is one. They're selective 5-HT1 serotonin reuptake agonists, so if he's on an SSRI they don't let you take them (possible serotonin syndrome?).
It's been great for me though. I never realized before how little relief I actually got. I considered that my OTC medication worked if the headache got better enough that I could function. It wasn't until I took Frova during a really bad attack, complete with aura, that I realized how little I had been settling for. The Frova even helped with the non-headache part of the migraine. It hadn't even occurred to me.
So stubborn insistence on not taking the heavy drugs has cost me a lot over the last ten years or so. :)
