yes, i've had to sign a few of those forms to either get treatment at no charge or to be released from a hospital.

i saw your ideation post and meand to find a dictionary to look it up, but i am stranded couch-surfing at the home of folks who don't own a dictionary. i imagine that it could be used in other contexts, but i've only heard it in one.
