My cat is sick and I'm miserable (bio functrigger)

I adopted an 18-year-old cat a year ago.
Anyway, he's very ill now and throwing up all over my house. But half the time he's kittenish and happy and half the time he's sick. It's been going on for months but gotten much worse recently.
I'm a total wreck and don't know what to do.
I just made a therapy appointment but I don't know what good that's gonna do me.
It's really hard to know when to put an animal down. When does his suffering outweigh his happiness? Where does my quality of life fit in? Right now it's in the f'in toilet.
