si and a pdoc

i see my first pdoc i 1 week(outside of hospital)
i am frightened if i tell him i havent stopped with the "si".
iam afraid to tell him i avoid my kids so i wont verbally abuse them.
i feel so bad that yet another ad wont help me.
i feel like i failed again:(
i know better-but i feel so bad.
