Wow, you have the same exact OCD I have-constant suicidal ideation despite euthymia.

This manifestation of OCD is often referred to as 'Pure OCD'-obsessional symptoms without compulsions.

I wouldn't liken your recurrent thoughts to counting slats on the venetian blinds. Those compulsions are usually carried out to ward off bad thoughts.

What you are experiencing is simply obsessional thinking. You have the bad thought and then you obsess on the bad thought.

Have you ever visited with a therapist who specializes in treating OCD with ERP (exposure and response prevention) and ACT (acceptance and commitment therapy)? If you want to deal with this obsessive thinking I'd recommend you do.

On the medication side, Zoloft should be helping with this type of thinking, but maybe its not as effective as it should be. Have you ever considered trying a different AD?

Another thing you can do is add an atypical antipsychotic, which will calm down activity in the nucleus accumbens, which is often overactive in those with OCD.

Adding Clomipramine is another strategy-basically Clomipramine is considered the best treatment for OCD.

Sorry for the barrage. I don't know what kind of impact the suicidal ideation is having on you, but I hope some of my suggestions make sense.
