Combining D2 agonists with anti-psychotics

Posted by JohnX on March 14, 2001, at 2:19:31

Anybody have any inclings as to what the effect of combining a dopamine D2 agonist like Mirapix with an atypical anti-psychotic like Zyprexa or Ziprasidone which have D2-antagonist activity? Would this have any implications on the possibility of getting tardive dyskinesia?

John,

Remember that the antipsychotics will at low doses be antagonizing the presynaptic autoreceptors while Mirapex stimualtes the post-synaptic response. So they are not, one would think conteracting one another. On person is combining rispedal with mirapex. My experience with the the combo of mirapex and amisulpride was an increase in fine motor coordination but not additional mood benefit. The 5-HT2 antagonism activitiy of antipsychotics (i.e. Zyprexa) may act synergistically with Mirapex by further enhancing certain dopamine repeptor functions. But this is just specualtion until more reports come in on such combos. I would think it prudent to trial an antipsychotic separately from the Mirapex before considering trying such a combo.

I was taking Zyprexa for about two years before I started on Mirapex around the first of this year. I did my research and both my pdoc and I thought there was some hope with Mirapex even though we both thought that the Z and M may cancel each other out and end up with nothing.

The good news is that they have complimented each other beautifully. I have had great success with both. The Zyprexa got rid of ruminating thoughts and GAD problems. In the mean time Mirapex has lifted a good part of my anhedonia problems.

I did go through the whole lot of stimulants before trying Mirapex (Wellbutrin, Adderall, Ritalin, etc.) I only stumbled upon Mirapex while researching Selegiline as an option. I thought Mirapex stood a better chance and we agreed to give it a trial. It's now three months later and I almost have a life again.

In short I haven't had any problem combining Mirapex with Zyprexa.

P.S. I also take Nortriptyline as my main AD and it still works fine.
