Why not just tell him? Its not doing you much good sitting on the feelings. What's the worst that could happen? Oh, he could reject you and turn you away as a patient, but I like to think he's the humane type. Would it hurt to find another doctor?
What if he likes you back? This seems like a much harder question to answer.
