In your head And taking a sexual history

What significance does it have when a T asks you if you're "in your head a lot"? He asked this, and then all week I kept thinking, why would he ask that, and what's the significance?

Also, B2 brought up the sexual history. Is that always done? Why would it be done, and at what juncture? I'm not sure if I can even remember my sexual history! I guess I don't have too much history, well not a very exciting history anyway.
