Just low dose naltrexone these days. As an AD, I find it only mildly effective. I've tried quite a few ADs, and the serotonin and/or norepinephrine route has been a bust for me. I do like dopamine, but I'm very sensitive to stimulants, so I can't tolerate it for long. A few years ago, I was taking the muscle relaxant baclofen for muscle spasms in my back, and it had the unexpected side effect of eliminating my depression and anxiety -- perhaps because it increases striatal dopamine levels. But after a week, it started to cause terrible insomnia, so I had to discontinue it.
Neurofeedback helped me somewhat. What I'm planning to try next is stellate ganglion block.
