Hi Zonked. I'm sorry to be so late to respond to your questions, but I have not been disposed to posting lately.

First of all, for me, higher dosages of prazosin feel like a very clean antidepressant. My only complaint regarding side effects is a suppression of my sex drive. I take 30 mg/day (10 mg t.i.d.). I followed the titration instructions found on the manufacturer's package label. You can find this on Rxlist with one important exception. The first dose should be 1 mg taken at night so that the initial reaction, which includes dizziness and syncope, does not occur during waking hours. The second 1 mg dose should be given the following night. Thereafter, one can begin dosing three times a day and titrating gradually. I found that the first few days of prazosin treatment produced mild dizziness, fatigue, weakness, and feelings of malaise. These things disappeared completely within a week. I also found that an antidepressant response can come and go at a given dosage, requiring continued titration. I should think that the initial target dosage should be 6 - 12 mg/day. I am currently taking 30 mg/day. However, I experienced a discernible transient improvement at 3 mg/day. Some doctors are bringing their patients with PTSD complicated by comorbid depression up to 40 mg/day.
