As someone who must take a neuroleptic in order to keep the batsh!t at bay, I'd say: lowest effective dose. This is what docs are supposed to aim for, but rarely do, especially now that the "atypicals" have taken over the market.
I think zyprexa comes in 2.5 tabs. I'd try those, personally, and go up in increments of 2.5mgs/day until you reach a pre-set max. I'd personally set the max at 10, since 10 is half the max fda approved dose and strong enough to sedate an agitated psychotic person.
What are you thinking in terms of dealing with the weight gain and potential for diabetes? Metoformin, maybe? I read somewhere that high-dose antioxidants, such as alpha lipoic acid and/or green tea extract, can help. I would imagine they'd also reduce the TD risk, but that's purely speculative.
