What's not "working?" I mean, what are you looking for? You seem to take high dose ADs and high dose zyprexa, plus some Nuvigil (Provigil? I forget).
Personally--and I know you're going to not like this--I feel as if a lot of your "issues" are related to home environment, finding purpose in life, and your shady past. I've been there and trust me--drugs won't get you out of it. When I was 20 and stuck living at home with my parents, the benzos and amphetamines didn't do anything except fry my brain and make my life even harder. Now, 7 years later, I take 1 medication (sometimes...) and I'm doing the hard work of getting things straightened out.
I'm not that special. You can do this. If a job isn't in the works, consider disability. At least you'll have your own health insurance and a little bit of income. I don't know if you're looking at SSI or SSDI. SSDI pays more (I don't know how much...), so maybe you could get a place or live with someone else? Just throwing ideas out there.
No doctor really holds the key for you. I mean, this doctor doesn't sound terrible, but its not as if she's going to OK amphetamines or anything. I really think cutting out some meds---the antipsychotic, the prozac--might help. If she's giving you namenda to help your damaged dopamine receptors, I cannot understand why she'd keep you on a high dose antipsychotic. Prozac isn't exactly great for motivation, either--it tends to flatten people out, especially at higher doses.\
If I had a prescription pad and could give you meds, I'd try to treat it like brain injury, or frontal lobe syndrome or "abulic depression." Wellbutrin, dopamine agonists, mild stimulants. 20mgs zyprexa? no way.
