Persephone, what a bind. I'm working with something similar. Persistent disabling fatigue. For myself, effexor was initially stimulating. So was Wellbutrin. That you've had a 15 year run with effexor sounds really good.
I am trying emsam right now. About ten days in. It is mildly stimulating--it feels gentle, which is good. It is helping my mood. There is still fatigue, but I do have health issues.
You've probably addressed this already, but a thorough thyroid assessment is warranted, including thyroid antibodies.
Fatigue of the magnitude you describe is difficult to imagine unless it's experienced :(
