3 months sounds like a long time for a drug to kick in...and then heading back down after 2 months? I guess if you felt better you might be in the right class of drugs - have you tried another SNRI like Cymbalta or a TCA with a strong NE effect like amitriptyline? The amitriptyline would also help you sleep.

About the anxiety - well, I'm not a big fan of Xanax long-term although some do very well on it. I especially don't like it for sleep. Don't know what you could do - if the psychotic depression gets better does the anxiety follow?
