I don't doubt that important differences may occur in some cases. I would, however, argue that the majority of reported differences between brands and generics are due to perception, not pharmaceutics. This is particularly the case for drugs which have a wide therapeutic index. A minority of drugs (such as theophylline, lithium and cyclosporin) have a very narrow therapeutic index and are best prescribed by brand.
