I may have missed it, but have you tried bupropion (Wellbutrin; Zyban)? It often works well in combination with a serotonin reuptake inhibitor (SSRI; SNRI). A good friend of mine is very much improved now that she is taking a combination of escitalopram (Lexapro; Cipralex) and bupropion.
