It sure as heckfire is effective for panic. At least as effective as Paxil (http://www3.interscience.wiley.com/journal/119945101/abstract?CRETRY=1&SRETRY=0). There have even been studies suggesting that a very low dose of clomipramine can be effective in this regard, such as http://www.sciencedirect.com/science?_ob=ArticleURL&_udi=B6TBR-3S84MBF-5&_user=10&_rdoc=1&_fmt=&_orig=search&_sort=d&view=c&_acct=C000050221&_version=1&_urlVersion=0&_userid=10&md5=59731222d8903c1dbbd5b5e10327b29e, which concludes: 'These results suggest that low doses of clomipramine such as 10 mg may exert anxiolytic effects during the acute phase of treatment in highly anxious humans.'
Thing is, you are on an SRI already (Effexor). Clomipramine is as potent an SRI as sertraline (Zoloft), which is very potent. (Effexor is a very weak SRI, about four times as weak as fluvoxamine (Luvox), which is itself the weakest SSRI.) I have myself been adding a low dose of clomipramine, 10mg, to fluvoxamine (Luvox) treatment. That worked wonders for me, although it should be taken into account that fluvoxamine inhibits clomipramine's metabolism.
