I would tell him now, personally, so that you don't subject yourself to a whole trial of meds you've already tried. Just tell him you and a member of the office personnel had repeated differences of opinion and you mutually parted ways. He will respect your honesty I think.
Then put together a comprehensive list of all meds, doses, etc.that you've tried, something typed up, formal looking, and give the new doc that. Just one way to go.
