It is an orphan drug -- that usually means fast tracked by the FDA for a very rare condition like hopefully some day ALS.It would not be used off label by psychiatrists unless there was such a rare condition that presented a need for it and orphan drugs are sometimes prohibited from being channeled away from those in need.
I'd say one exception is tetrabenazine (Xenaxine, Nitoman), which just came to the US (its been in Canada and England) for Huntington's Chorea. It has been tested at Baylor College some years ago for TD.It may be of use for some people with TD, although it can cause it too as well as TDy, but mainly it may leave the patient with a permanent or semi-permanent pseudoparkinsonism.
Its better than its old family cousin Reserpine, another dopamine depleter (not quite like a dopamine blockade like APs).-- tidings
