I would second just going as you are, whatever state you happen to be in. I learned my pdoc goes by how you look - he only started taking the possibility of me being depresed seriously when I turned up unshaved with dishevilled hair. Apparently if you've made the effort to get washes then you can't be depressed.
They live in another world. Just go as you are, stinking, sweating, smelling, whatever. They'll have to put it in the report if you come in a mess.
