The information I read about Emsam showed that Somerset had done research on the 9 and 12 mg patches, as well as the 6 mg. There research showed that the 6, 9 and 12 mg patches were equally safe. The safety was quite substantial, but there were variations in response between patients and in the same patient at different times.
Some, quite rare, people did have tyramine reactions. It was found that the risk was essentially the same on all dose levels-- and that therefore, there is some, very small risk for susceptible individuals on the 6 mg patch. The arguments for requiring warnings on the 9 and 12 mg patches didn't make much sense-- but there they are.
So you really can't say never-- although it really would be extremely unlikely.
