I don't know of any reason why you would not want to target 45mg or higher of Remeron. Just watch for serotonin syndrome (a seemingly paradoxical effect from such a combination) as there are several anecdotal reports on Medline regarding the combinations of Remeron with an SSRI or lithium.

Wait for some more responses. Hopefully, someone will have personal experience with the combination.
