My pdoc will sometimes make use of nortriptyline when the SSRIs and/or Effexor have failed to get the job done. This is a norepinephrine selective reuptake inhibitor from the tricyclic class. It is relatively innocuous for a tricyclic and is widely used by GPs for things like headaches and chronic pain. Cymbalta might also be worth considering, I suppose, particularly if you can just try some samples. Nortriptyline is so cheap there is no need for free samples and is affordable even without insurance.
