I'll second the call for upping the Effexor, since I found it very effective at reducing my anxiety when I started it, although it didn't do much for my depression. We added Prozac for the depression, just a small dose (10 mg), and that seemed to work.
The bad news is that it only worked for a couple of years before the breakthrough depression became full blown depression, and then I had the fun of withdrawing from it.
The good news is that that was just my experience, and your mileage WILL vary.
Talk to your doctor about this. See if maybe you're dealing with Effexor poop-out, and maybe refer him to the link below about ketoconazole. I've been reading up lately on drugs to augment ADs and reduce cortisol, and it seems as if a lot of doctors are researching the same thing lately.
