Survey - Those with comorbid SP/ADD?

I am wondering what drugs and at what doses the people here with co morbid Social Anxiety Disorder and Attention Deficit Disorder take to feel "normal."

The questions:

1) Do you label your Social Anxiety as severe, moderate or mild? Is it general or specific?

2) In a sentence, how does your ADD/ADHD affect you? Example - I am inattentive, lazy, unmotivated, etc.

3) What do you consider to be the most debilitating? What do you have the most difficult time controlling? Example - ADD/ADHD/SP

4) What medicines do you (should you) take? What doses and at what times? In theory, how does each medicine help?
