I think you should first explore increasing the dosage of Effexor to 300mg if you haven't already. In addition, I don't think Remeron is adequately explored until one gets to a minimum of 45 mg. Combining Effexor with nortriptyline is an excellent idea. It helped me. The trick with nortriptyline is that you must get the dosage just right. Too high is as bad as too low. Be sure to test your blood levels in the beginning to fine tune it.
