Nardil stop working again at 60 mg !!

hi everybody
the first time Nardil pooped out was at 45mg after 6 weeks. So my doctor told me to raise the dose to 60mg . Now after 5 weeks on 60mg last day and today I felt very bad , a lot of fear , the Nardil effect is gone again =( . In The first week on 60mg I had one incredible amazing day , the best day in my last 8 years of my unhappy and feared life , but I only had 2 days like this in all this 5 weeks on 60mg.
I dont know If Im right calling this a poop out of the medicine
All I know is I want this thing to go away.
I couldn´t talk to my doctor today , and Im willing so much to increase the dose to 75mg , but i dont want to do it before my doctor tell me to do that.
Im also very afraid of this "poop out" ,because is this happened at 45 , then at 60 ,what could tell me that this is not going to happen at 75 or 90 mg? =(
