Derealization, in a lot of cases and mine, is where your brain switches to an unreal feeling/perception of the world. You know that a dime is a dime (ie your not psychotic) but you FEEL unreal, or the world feels unreal. I have derealization seizures which are like panic attacks but the main symptom is derealization. They are like seizures and I lose control and run in fright. Extremely scary, I feel I will never return to my normal state of consciousness. It sounds like your nephew is suffering plain paranoia.
