Why your brain poops out on dopamine or whatever is
to complex a question. The big picture question is why
have you gotton the erratic responses across such a broad
array of medicine.

Usually AED (antiepileptic drugs) aka mood stabilizers
or lithium augmentation would be the next step.

FWIW, Topamax removes my tolerance to stims and alcohol.
But that's just me. Klonopin , another AED, also removed
my torerance to stims (But I grew tolerant to it). My problem
for a long time was that I had undiagnosed Bipolar. The hypomanic
symptoms were less clear, but the medicine patterns like the one
you describe are similar.

AED's and lithium stabilize your brain's feedback mechanisms
to keep things from getting locked in one direction or from
oscillating out of bounds.
