If your TSH is above 4 (hypothyroid), almost all docs will give T4 alone.

If its below 4 (ie normal thyroid aka euthyroid) supplementation is generally not done (osteoporosis risk) except that some docs will do it to augment AD's. I think that most will just give T3 (Cytomel). But some give both either in natural animal extracts (Armour) or synthetic (Thyrolar). Some patients feel better and some feel worse so its just a trail and error thing.
