I totally understand where you're coming from. I would talk to your pdoc about the atypical antipsychotics (Zyprexa, Seroquel, Risperdal, etc.). I have taken Seroquel 'for sleep,' with the added benefit of helping with suicidal ideations. And Seroquel doesn't have as high a risk of weight gain, though I can't speak for the others. I tolerated it pretty well...just a thought...
In addition, my pdoc talked to me about how it's possible to have 'too much' medication in your system to the point of where it doesn't really work the way it should. In my case it was Paxil, where my former pdoc had me on 80 mgs with 300 mgs of Wellbutrin SR. My new pdoc said the Paxil, which started working but then stopped (hence the raising of the dosage level) was probably at a dosage above my therapeutic level, so it wasn't doing any more good than if it had been below that level. More is not always better, he said. I don't know the specs on your drugs, how they're metabolized, etc., but that's something else you might talk to your pdoc about, especially considering the Effexor/Zoloft combo. I had a bad reaction to being on too much Paxil, including increased anxiety & irritability, even though it's approved to treat anxiety!
