Mixing neuroleptics and lithium may increase the risk of neurotoxicity (including extrapyramidal symptoms), yup. Promethazine isn't a real neuroleptic, though - it has little effect on dopamine receptors, and as such rarely causes movement disorders (and isn't a very good antipsychotic).

So I guess the answer would be, "maybe, but it seems like a stretch."
