Generally when a person has a partial response to a medication, the first strategy is to increase the dose to either the maximum recommended dose (for Paxil that would be 60-80 mg and for Remeron about 60 mg) or until side effects become intolerable. If you weren't having problems taking the Paxil or Remeron I probably would have increased the Paxil and later the Remeron if you were better but still not in remission. After that we'd have to talk about what symptoms in particular were still returning and see if there's a medication strategy that would specifically address those symptoms. As far as a dose based on weight, there really isn't one except for children. Some little tiny people require huge doses of medication and on the other hand very large people will sometimes respond to tiny doses of medication. It's really not body size that determines response to a psychiatric medication, it's the degree of chemical imbalance in the brain.
