I feel that my problems are deeper in my brain (like the brainstem). I have a lot of sleep / wake issues.

If that is the case, how do you adress it as opposed to a problem that is not situated in the Brainstem?

I don't know. Have you tried melatonin? Some people swear by it.
Most people don't do well without routine. I don't have a regular sleep-wake cycle without routine. I used to work at a horseracing stable and needed to be at work at 5am. You get used to it. Difficult for the first week or two but you do it because you don't have a choice, really. Then would find myself falling to sleep faster and more soundly and waking up just before it was time to get up. Same when I needed to get up to get to class at 8am or whatever.
I try and mimic that for internal reasons but I don't seem able to do it. I'm not competent to make a decision about whether it's time to get up or not when my alarm goes off and I don't physically have someplace that I need to be.

Yeah, waking up is really tough for me sometimes. I somehow manage to turn off the alarm and continue sleeping without having much conscious control over it.

I'm not sure exactly how to address issues specific to the lower brain. I have always felt that with more stable sleep, my mood disorder may just vanish. This past week I literally started sleeping every other night (like clockwork). I've heard of some mood disorders spontaneously switching to a 48 hour cycle. Even with 50mg of seroquel + 1mg of lorazepam I could not fall asleep. There was no mind racing, just this feeling that I was not allowed to sleep.
