It would be a good idea to research Saphris (asenapine) further. To the best of my knowledge, Saphris is not known for having an exceptionally higher rate of EPS than the other AAPs. The lowest rates of EPS are with clozapine and olanzapine. Anything else will be higher by comparison. You may want to compare the manufacturers' labels or monographs, and compare EPS for yourself. I have only seen a handful of people on Saphris, but none have developed EPS. If you find evidence to the contrary, please post it.

well for now i am trialing Luvox. I think i will try higher dosage. Currently 25mg. Not feeling as well anymore.

Tonight i take 50mg

As I have mentioned, Luvox (fluvoxamine) is useful to treat psychotic depression.

I would gradually push the dosage of Luvox as tolerated. It often occurs that one will experience an initial blip improvement within the first 5 days that soon disappears. However, this frequently proves to be an index of good prognosis. Let's hope...

The recommended starting dose for Fluvoxamine Maleate (fluvoxamine maleate) Tablets in adult patients is 50 mg, administered as a single daily dose at bedtime. In the controlled clinical trials establishing the effectiveness of Fluvoxamine Maleate (fluvoxamine maleate) Tablets in OCD, patients were titrated within a dose range of 100 to 300 mg/day. Consequently, the dose should be increased in 50 mg increments every 4 to 7 days, as tolerated, until maximum therapeutic benefit is achieved, not to exceed 300 mg per day. It is advisable that a total daily dose of more than 100 mg should be given in two divided doses. If the doses are not equal, the larger dose should be given at bedtime.

I don't think there's anyway to know until its been in use for a while. Plus, even then...its hard to estimate the TD risk for an individual medication, because people switch meds often and are also often on cocktails w/ meds that may increase the TD risk (antidepressants, stimulants, lithium, etc.).

EPS rates also aren't always the best indicator. Abilify has relatively low EPS, but it causes TD and the risk is higher in children and adolescents. Case reports have popped up from the lower doses used to augment treatment in unipolar depression (scary, huh?). Risperdal is OK with EPS up to about 6mgs, then its a nightmare.

I guess I'm saying...there's a lot of variables here. I personally take b vitamins and high dose antioxidants (Orthomolecular) in hopes of preventing TD. I've been on Abilify over 5 years @ 30mgs/day, not a twitch, not a tremor, no tics...good times.

Anyway...I hope things work out for you. Dropping down to 1 antipsychotic would probably lower your TD risk.
