Schizotypal PD -- best medication?

Hi,

I'm a 33 yo male from Lisbon, Portugal.
I was diagnosed with social phobia when I was 19 and started taking paroxetine 20 + buspirone then. It helped with my severe social anxiety but the fatigue was too much to handle.

When I was 23, I was diagnosed with schizotypal personality disorder. I agree with the diagnosis. Fortunately, I don't experience hallucinations and I've never had a psychotic crisis. Even my social anxiety is not triggered by paranoid ideation; it is rooted on my own body image insecurities and fear of rejection.

My problems are related to anhedonia, procrastination, lack of motivation and being very self conscious.

I've taken a lot of antidepressants, sometimes with low dose antipsychotics in the past 14 years.

My current psychiatrist thinks medication won't help much. I'm not currently on meds.
I actually agree with him. And I like him a lot because he listens to me. He seems to care and that makes a difference.


However, I would like to know which medication would you prescribe. I'm aware the use of meds on personality disorders is controversial: I am interested in minimizing some symptoms, I know current meds can't "cure" personality disorders.

I wouldn't treat your personality disorder with medication.

If you had depression accompanying it and/or psychotic behavior I would prescribe antidepressants and/or antipsychotics.

Hi Eric,

Thanks for the reply and professional advice.

Fortunately, I've never had psychotic behaviour.
I've taken low dose antipsychotic in the past to augment antidepressant efficacy and it worked. However, the side effects were too severe (sedation, loss of libido) and I stopped taking antipsychotics.

And then I slowly discontinued the antidepressants.

I'm not taking any meds, now.
I really feel better than I've ever felt. I think running almost daily is helping me a lot, I must say.

However, social anxiety and a bit of depression are still present. Moclobemide, amisulpride, Effexor, ... Would it be a good option?

Social anxiety can be difficult to treat however there are options. I would recommend an antidepressant, but I would augment treatment with Topomax, which has some case studies backing it as an effective treatment for social anxiety. Keppra is another drug that has shown promise in the treatment of social anxiety.
