MAOI taken at night - Nardil better suited?

Posted by caller on April 15, 2013, at 4:34:06

Hi everyone,

I've been on Parnate for about half a year now and the only thing that works for me is to take it at night.
If I take it during the day it'll make me completely useless and manic.
I've tried with NRI's, bupropion, amphetamine, caffeine, modafinil, nicotine etc etc to fix this but it just doesn't work.
I think this weird reaction may be related to excess tonic dopamine, as I react very similarly to selegiline and 5HT2-C antagonism.
However, taking it at night works fairly well but seems to affect my sleep quality a lot. Hence, my question is if you think a switch to Nardil may be beneficial? (taken at night that is, I tried taking Nardil in the morning for a short period of time and it didn't work at all).

Thanks,
Caller

How much Parnate do you take?

How many doses have you been taking during the day?

If it made you MANIC how was any of that (NRI's, bupropion, amphetamine, caffeine, modafinil, nicotine) supposed to help?

Have you tried single dose right after waking up, seems to work better for some people, if you can tolerate a single dose that is? Have you tried as many divided doses as possible, every 2 hours?

Parnate at night didn't work for me at all, I would wake up 2-3 hours later with insomnia which would not go away for the rest of the night.

Sorry, manic wasn't the right word - severe akathisia would probably be a better description. That, as well as drowsiness, worsened ADHD symptoms, memory issues, cognitive impairment, feeling flat and hypotension are the reasons I don't want to take Parnate during the day.
Taking it all at once in the morning, as you suggest, does help but the problems are still there.
I've tried 10 - 75 mg and it gets worse the higher I go. Right now I'm on 20 mg but I'm starting to feel depressed so I'll be bumping it up to 30 mg. Taking higher doses during the day feels very dysphoric.

Taking it at night barely works for me either.. Taking it 4 - 5 hours pre-bed and then putting myself to sleep with antihistamines, melatonin, zolpidem and magnesium almost seems to cut it but most of the time I'll still wake up once or twice during the night..

This is why I'm curious if anyone have had more success taking Nardil at night/in the evening.
OTH I'm worried Nardil would deteriorate my cognition even further.. Not sure what to do really! :/

I take my whole dose of Nardil an hour or two before bedtime as it helps me sleep.

I'm sorry but I can't help you with any information on Nardil other than by saying that it has a different side-effects profile than Parnate and it's something that should be considered when switching from Parnate to Nardil.

Here is something I read years ago that stuck with me (I'm not sure this is the specific article I originally read it in, Cole may have written this point elsewhere, too). I can't get copy and paste to work, but here is a free full-text link. http://www.jfponline.com/Pages.asp?AID=515 When using an MAOI: "Attempts to alter this odd cycle [of being drowsy in the afternoon but then having insomnia at night] by changing the timing of the administration [of the MAOI] have generally not helped." They go on to recommend adding a stimulant, or nighttime Trazodone.

If the link goes dead, the journal publication info is: MAO inhibitors: An option worth trying in treatment-resistant cases. Journal of Family Practice, Vol. 1, No. 6, June 2002. By Jonathan Cole, MD, and Alexander Bodkin, MD. [They teach psychiatry at Harvard Medical School.]
