If I owned a pharmaceutical company I would have my chemists manufacture a 24hr slow release pill with either amitriptyline or nortriptyline + bethanechol.
E.g. 100mg amitriptyline + 50mg bethanechol

Do you dose your TCA more than once a day?
As far as cholinergic side effects, you could try green tea. It inhibits acetylcholinsterase for more than a few days per dose.

Did not know that, thanks. I recently made a switch from 100mg amitrip to 75mg nortrip, was partially responsive to amitrip. But before pushing the dose I decided to try nortrip again, it may work better with less side effects, can always go back to amitrip.
Can't say I had much side effects on amitrip, the orthostatic hypotension was the most notable, no dry mouth.
But you can be sure the ACh side effects will appear when the dosage increases./tensor
