I have been in hospital for 1 week now and today I am finally in the special depression section. I had a talk with the pdoc here and he has not come up with a final proposal yet but I imagine he will suggest to up my dose of Nort to maybe 150 mg - I am currently doing 120 mg and this brings me just inside the therapheutic window, to avoid too many SE I have never gone higher. He has also mentioned to add Lithium. (I have earlier tried Lamictal as supllement but couldnt sleep on even on a small dose of 75 mg and it did not improve stability). I am unipolar and normally cycling within weeks/days between normal and down - NO mania. This time however it never cycled back to normal.
My thoughts:
Could upping the dose of Nort alone bring me out of this dep episode? Pdoc says higher in the therapeutic window => more likely to get relief
Would adding lithium really help?
What should be tried first? Nort up or adding lithium?
I am also taking 15 mg Mirtazapin and 37,5 mg Valdoxan.
All in here are the "problematic" cases, many are doing ECT, they claim it really helps...

Drop the Valdoxan and add 100mg sertaline (Zoloft)
Increase the nortriptyline based on blood levels
If this doesnt work, add lithium
