i've been on emsam 9mg for 3 weeks and it's not doing much, particularly for my worst side effect, derealization. I don't have the patience to wait any longer; I feel that if the 9mg isn't doing much then 12 won't either. I'd like to switch to Nardil, which has worked for me in the past despite the awful side effects, because I've tried everything else. Is there a washout period? I don't have the patience to wait to titrate up, can i just start at 45mg?

McLean docs insisted on a two week washout. Don't know how other docs feel.
