The best way to treat akathisia is to discontinue or significantly reduce the dose of the drug which is causing it.

If the offending drug is absolutely necessary, besides propranolol, what other remedies are available? I have read things suggesting that clonidine, mirtazapine, and cyproheptadine are effective.

Gabitril treats my Epilepsy, treats my anxiety and treats my ocd. Probably provides some mood stability, too.

When I had bad akathesia from 36mg of perphenazine, my doctor told me to take benadryl and that worked right away.

I don't think it makes sense to combine a dopamine agonist such as Requip with a dopamine antagonist (antipsychotic). It would be much better just to reduce the dose of the antipsychotic.

I have read things suggesting that clonidine, mirtazapine, and cyproheptadine are effective.I think akathisia is a sign that the antipsychotic dose is too high ie. excessive dopamine antagonism is present. Akathisia is very distressing and can lead to an exacerbation of psychosis. Benzodiazepines such as diazepam and anticholinergics such as diphenhydramine can be used temporarily.
