What would be a good TCA to add???

Posted by Roslynn on May 23, 2010, at 17:20:50

Hello everyone,

My pdoc has me on protriptyline, which I'm not liking so far..it's making me tired and irritable even though my doc said it's supposed to be activating.
What would be another choice of TCA to add?
My current meds are:

Prozac 20mg
Lithobid 600mg
Ativan 1-3mg prn
Seroquel 125mg
protriptyline 30mg

Any thoughts on which TCA (or other med) could be added?
I have already tried desipramine and nortriptyline.

Thank you for any help!!

Roslynn

it looks like you've tried them. if you want something activating, maybe modifinal or yohimbe - for yohimbe, you could probably just take more seroquel instead; at that dose its just an antihistamine. pretty similar story with mirtazapine.

Prozac + TCA: Keep Dose Low!? Trimipramine?

Prozac inhibits the breakdown of TCA's very strongly, so maybe your dose of protriptyline is simply too high.

I found imipramine to be a cool add-on to Prozac in low doses (no higher than 25mg).

You could also try trimipramine, an atypical TCA with good antidepressant qualities which improves sleep architecture while intensifying REM-sleep (usually, AD's suppress REM-sleep.)
