I promised a neighbor I'd post this. Okay she takes xanax and has for years. About to start a very stressful year of teaching her 24th one. She saw the pdoc here I also saw and he's been letting her take the xanax as she's taken for years. Well she couldn't sleep so he prescribed resperidol .25mg. She took it and was up til 4 am and angry when dealing with people at school. She has horrible anxiety and restlessness called the doc he said she had akethesia but said skip a night and take it again. Needless to say she's not but why would a pdoc recommend that someone with akathesia and inability now to sleep when she was sleeping okay til the stress encourage her to stay on the med that causes akatheia his diagnosi. That's a bad idea. Akathsia means there's too little dopamine. encouraging this is really bad practice.

Why dosn't he just prescribe her a proper sleep med or something similar. Psychatrists seem to be so fond of prescribing anti-psychotics,

Ask if the doctor might prescribe something like trazodone or doxapin. If she tends to have periods of marked irritability zyprexa might be a better option.

Seriously she's a remarkeable woman and was shocked to see what he'd ordered. Her xanax at lower doses worked just fine til he added the resperidol. Funny thing just home from my pdocs all day trip. PA still filling in since her accident. Get this she still wants me to up my xanax actually almost pleaded with me to take during the day. Our deal I'd consider it. I like this lady. Phillipa

Not quite... it means where the dopamine blockade is taking place for whatever reason is spilling over into the wrong pathway, principally the nigrostriatal system.If I had the ability these days to read much, Stahl is a good source of information.
As for the discussion, I also don't see the purpose of Risperdal for sleep... low dose Seroquel, if an AP is to be used makes much more sense, at least to me.I know Risperdal gave me bizarre nightmares.
Low dose amitriptyline (10mg) would be better. Doxepin possibly but it isn't terribly strong and wears off (at least in my experience).
Or doxylamine (Unisom doxylamine formation OTC).

Final outcome is upped dose of xanax and restoril for sleep at 30mg. Love Jan
