also wondering if they mean indirect agonism, by increasing dopamine or dopamine sensitivity, or direct, by memantine having actual affinity at the receptor.

The D2 receptor can be pre- or postsynaptic.
The presynaptic receptor has modulatory functions. The postsynaptic receptor has excitatory or inhibitory effect.
Many neuroleptics have modulatory/inhibitory effect on the D2 receptor.
