Doctors feel they have all the tools. As such, they feel at liberty to make the unlogical assumption that a treatment resistant depression must necessarily be bipolar.
I feel the main reason behind this is so they can extend their drug aresenal, while (in their eyes), maintaining the integrity of their whole system.
