I have no idea how it works in Canada. So I'll tell you how it has worked here in the States for me.
When I have switched docs or moved, I had to request my documents from my past doctors for them to have that information. In my HMO, my psychiatrist has automatic access to my medical records, but my doctor cannot access my psychiatric records. So, while I don't know how it works in Canada, and I don't know if you're talking about within a healthcare system or between totally different docs, that is how I have had to keep my doctors in touch with each other.
