My mother is coming off 150mg mellaril and onto 5mg of zyprexa. She wants to do it all in one night, but I am not sure.
What risks are there in combining these two, and is a direct swap advisable ?

I know there was some discussion above, but the real question I have is weather combining the two drugs is ill advised ?

Tapering the mellaril as low as possable then do a direct swich.
Though I am not confident zyprexa will be the one, that she will stick with.
I see her having problems with the increased appetite.

hey! About the increased appetite...apparently, there's been some success with using Meridia as an add-on (I think at the beginning of Zyprexa therapy). Patients either didnt gain weight or were able to lose weight and the neuroleptic potency wasn't adversely affected. Using an older med, like phentermine or phendimetrazine, might be helpful as well. If she doesn't stick with zyprexa, risperdal or geodon might be good, since they cause weight gain less often than zyprexa (geodon doesn't really cause weight gain all that often in general). If she's using an atypical for agitation and what not, you might want to add hydroxyzine or something if you opt for a less sedating medication, though it may not be necessary. Good luck!
